Treatment with my local sleep medicine doctor has gone as it can, without any resolution. I have the two severe types of sleep apnea that cannot be treated due to a type of sleep disturbance, and I'm ok with that. But my specific sleep disturbance, ISWRD (irregular sleep wake rhythm disorder), does not seem to be resolvable locally. As background, this is most frequently seen in the elderly population in nursing homes, and thus I'm an outlier.
Would anyone happen to know of a specialist or facility that might be known for addressing ISWRD? Anywhere is fine as long as I can do telehealth.

That paper stated that there are strong indicators that ADHD could be the symptom of a circadian rhythm disorder (but more research needed, so take it with a grain of salt).
However many of us with ADHD have irregular sleeping pattern. Most people I meat so far kinda just go with it though. What are the actual issues you face?

@agowa338 Hi and thanks for this information. Yes, I am an AuDHDer. I will look for that paper, as I didn't know there was a possible relationship.
The issues I have is that my to-sleep and to-wake times vary wildly from day to day. And when I do sleep, it is for 15-60 min and then I am up for hours, and then that might repeat a few times over the night (or maybe not). And sometimes I have daytime sleepiness requiring a mid-day short sleep.
I had to discontinue sleep apnea treatment because insurance compliance requires a minimum of 4 hours of sleep 70% of the time. I am lucky to get 2-3.5 hours.
It'd sure be nice to improve on this, thus my call out for a possible specialist person or facility.

Well it takes time to find the right medication and dosage for you.
Methylphenidate (Ritalin/Medikinet/Concerta) is not the only ADHD medication. If it isn't the one for you (e.g. the highest dosage is not effective enough) they move you over to others like e.g. Lisdexamfetamine (Elvanse/Vyvanse/Livizux).

I don't remember her first name but Dr Cook at Optum Healthcare in Arlington, Washington, has dealt with a number of cases like this. I had multiple tele-health visits with her post-COVID, but my first visits were face-to-face and pre-COVID.She worked very closely with a couple of the local sleep centers to best map out patterns and treatments.

RP2040 Powers A MIDI-Controlled Soundboard
When you’re livestreaming, it can be tempting to fire off all kinds of wacky sound effects like you’re a morning radio DJ back in the heady days of 1995. If that’s who you want to be, you might like this soundboard project from [Biker Glen].
The build is based around an RP2040 microcontroller. It’s paired with an I2S digital-to-analog converter for sound output, which in turn feeds a small amplifier hooked up to a speaker or a line output. The RP2040 is programmed to respond to MIDI commands by playing various sounds in response, which are loaded off a microSD card. It’s able to act as a USB MIDI host, which allows it to work seamlessly with all sorts of off-the-shelf MIDI controllers like the MIDI Fighter or the Novation Launchpad.
It’s an interesting hardware solution to a problem that you could probably also solve with software on your streaming machine, especially if you’ve already got a USB MIDI controller. However, there’s something to be said for lightening the load when your streaming computer is already doing lots of hard work to truck video up to the cloud already. Files are on Github if you’re eager to replicate the build.

I made it!!It was not even the save pod.
In the end I've smashed the corvette a bunch of times against some asteroids, then a few freighters came, I've shot at one and let the sentinels shoot me down.
I died, earned 1 point as pirate, but in the end I woke up back on the space station, outside that frigging corvette!!
