@ccohanlon If Palermo doesn't do it, try south.
Agrigento is nice. Friendly locals. South has much warmer climate in winter than north of the island (big mountains). Less Mafia, too. Good hospital.
Or Licata. Can keep the boat in the port. Bit more mafia, but nothing compared to Palermo. Town is dirty and run down but it grows on you. Fresh fish straight off the boat for nothing prices (doesn't count in the catch quota). Cheap shuttle to Catania airport. Hospital was unbelievably shit though (googled my ER doctor - convicted child molester, check!). The good cardiologist only comes down from Parma every couple weeks.
Bunch of retired expats in the area. Friendly hiking club. Great food. Prices much lower than mainland. Summer too hot and there's no water.

I actually know Sicily reasonably well. None of its SE and S interest me (apart from their history) and given my checkered past, the mafia isn't among my concerns.
We'll see how we feel at the end of our fortnight here.
