A lot of readers on multiple platforms have let me know they didn't get this.
He's my thinking:
The halberdier (bodyguard soldier holding a huge axe) is having a stroke, (heart failure). Drooping face is a telltale sign.In a moment, he'll collapse forward, bringing the halberd down on the newly crowned king, killing him.
But! A stroke is not a heart failure, so the subtle clue I felt clever about is actively misleading.
I failed to do my research.
